This applies only to graduates of CAPTE-accredited programs. All others must submit an educational evaluation.
For graduates of Texas CAPTE-accredited programs: Your program director will send a Letter of Completion with a list of graduating students directly to the Board. You do not need to send a separate Letter of Completion form.

Application must be approved before taking the examination. Please submit as soon as possible but no later than the dates below. After approval, candidates may register directly with the testing service.

Direct Access Patient Attestation And Medical Release Form
New legislation, SB 1187, passed during the 2021 General Assembly and effective July 1, 2021, extends the time allowed for a qualified physical therapist to evaluate and treat patients without a referral under certain circumstances from 30 to 60 days. Virginia Code § 54.1-3482.
With this change, the Board has revised the Direct Access Patient Attestation and Medical Release Form, also accessible on the Boards Applications & Forms page.

The Texas Board of Physical Therapy Examiners is a participating state in the Alternate Approval Pathway for applicants from accredited programs for licensure by examination. The AAP separates the functions of exam registration/approval from licensure application/issuance. The Federation of State Boards of Physical Therapy has the final authority to determine who is eligible to sit for the National Physical Therapy Examination with input from the schools, and the PT Board concentrates on the requirements for issuing a license once the NPTE is passed by an applicant.
The AAP does not affect the current procedure for processing foreign-educated applicants.

The application fee must be submitted in the form of a domestic cashier’s check or a money order and made payable to the Louisiana Physical Therapy Board. A personal check is not acceptable and your application will be returned.
A license issued to an applicant for the first time shall be for a term of one year or two years, to be determined by the birth year of the applicant . THIS FEE IS NON-REFUNDABLE.
