It is essential to realize that while most companies require a random drug test, most do not need a drug test for your new position. Many new businesses dont require a drug test at all.

What Drugs/substances Are Tested For With A Dot Test
The following is a list of substances that DOT drug tests check for:
- Opiates/morphine
In 2015, marijuana and amphetamines were the most common substances found in DOT drug test results for commercial motor vehicle drivers. Nonetheless, positive results remained low for commercial drivers, with just 1.85% of the total number of DOT test results. However, the department is still pushing to see a lower number to ensure transportation safety across the country.
Who Needs A Dot Physical Exam

A DOT physical exam is required by the Federal Motor Carrier Safety Administration for all commercial drivers.
Commercial drivers include drivers of vehicles that carry more than fifteen people, or you are paid for vehicles that are expected to carry more than eight people.
It is also required for drivers of vehicle that carry dangerous or hazardous materials that normally warrants the vehicle to carry a warning sign and for interstate drivers of heavy-duty trucks with a combined weight of more than 10,000 pounds.

We have a provider who in the past worked in a facility that whenever a driver has a history of previous illegal substance abuse they gave them a drug screen with every physical in order to certify.
Today we had a driver that admitted to previous use and refused a drug screening for his physical.
Do you have any thoughts? I am stumped as I never have had a provider drug screening during a physical without the request of a company.
If the examiner decides they need such a test in order to qualify the driver and the driver refuses, the examiner may disqualify the driver and report it to the national registry. This would cause the FMCSA to investigate should they then receive a qualified entry shortly thereafter from another examiner who may not be made aware of the drivers history.
The DOT physical exam requirements do not include a drug test, only requires a a urine sample. This is used to screen kidneys for potential medical conditions.
There is no Federal prohibition resulting from a driver testing positive on a non-DOT drug test. 49 CFR §40.13 requires that DOT and non-DOT testing be completely separate.
This is true for DOT drug test under CFR 382.

Which Jobs Are Required To Undergo Dot Drug Tests
Albeit all companies have the option to have their employees tested for drug and alcohol use, some are mandated by law to do so. Anyone deemed within DOT regulations as a safety-sensitive employee is required to undergo DOT drug testing. If you hold any of the jobs listed below, you are considered a safety-sensitive employee.
- Federal Aviation Administration Aircraft dispatchers, flight crew/attendants, ground security coordinators, etc.
- Federal Railroad Administration Hours of Service Act personnel, engine & train workers, signal service workers, or train dispatchers.
- Federal Transit Administration Vehicle operators, controllers, mechanics, and armed security.
- Pipeline and Hazardous Materials Safety Administration Emergency response workers, maintenance, and operations crew.
- S. Coast Guard Crew members who operate a commercial vessel.
What About Prescription Drugs
You may use prescription drugs and pass the drug test if you meet certain conditions stipulated in the Code of Federal Regulations , which are lists or schedules of controlled substances, such as opiates, opiate derivatives, hallucinogenic substances, depressants, and stimulants.
Non-Schedule I drugs may be used under the following exceptions:
- If a licensed medical practitioner who is familiar with the drivers medical history and assigned duties has prescribed the drug and
- If they have educated the driver that the prescribed substance or drug will not hamper the drivers ability to safely operate a commercial motor vehicle.
If the medical practitioner prescribes the medication without insight into the assigned duties, this renders the driver medically unqualified and the abovestated exceptions do not apply, even though the driver might have legally obtained the medication.

Dot Physicals: Do They Include A Drug Test
If youre a commercial motor carrier, then you know the drill: at least once every two years you need to have a DOT physical exam to renew your medical card. These exams are conducted by certified medical examiners whose job it is to evaluate you for a wide range of medical conditions and ensure youre healthy enough to safely operate a motor vehicle.
In addition to a general physical examination, your DOT physical will include a urinalysis. But unless the examiner requests a second sample be given for a DOT drug test , the urine sample you provide during the exam isnt used to detect drug use but to screen for medical conditions such as diabetes instead. Failing A Dot Drug Test

Unfortunately, some people are going to fail drug tests. It happens. If you fail a conventional pre-employment drug test, youre going to lose the job opportunity. Most companies will not hire people whove failed drug tests.
In some rare situations, they might give you another chance. The company may work with you and give you another opportunity once youve completed a course.
If youre dealing with a DOT physical and drug test, youll have to jump through another hoop too. First, you have to remember that your potential employer asked for the drug test. That company likely wont hire you now.

What About Prescribed Medications
While the standard DOT physical urinalysis doesnt screen for drugs, you will need to speak with the medical examiner concerning any prescribed medications youre currently taking. If you have a condition, such as heart disease, neurological or mental conditions, diabetes, high blood pressure, or have recently undergone surgery, you will need to disclose any medication that can possibly impair your driving.
There are certain medications that may prohibit you from operating a CMV while youre on the drug, although this decision is left to the judgment of the medical examiner. You will usually need to bring with you the prescription and a note from your specialist or primary care physician, indicating the potential effects of the medicine. It is then up to the medical examiners discretion to give you the green light to obtain a medical certificate.
However, if the medical examiner feels that your ability to drive safely on the roads is affected by your condition or the medication used to treat it, then you may be temporarily disqualified.
After youve weaned off the prescribed medication, you can then go back for a follow-up exam and be re-evaluated. There are cases in which you will be issued a medical card. However, it wont be valid for the typical 24 month period, and you may need to be reexamined in shorter time increments.

The DOT physical requirement is determined by your employer, but you will likely need to have one done if you meet the following criteria:
- You are paid to operate a vehicle that carries over eight people.
- You operate a vehicle that carries over 15 people, such as a bus.
- You operate a motor vehicle for commerce between states, and that vehicle has a gross combined weight of over 10,000 pounds.
- You transport hazardous materials in a vehicle that requires a placard.

What Does A Dot Physical Consist Of
A DOT physical is fairly consistent with any other type of physical examination performed under the care of a physician. Youll begin by disclosing your medical history and outlining any prescribed drugs that youre currently on.
The medical history component of the exam consists of:
- Prescribed medications
Once this is completed, youll move on to the physical exam.
The physical exam will check various functions of your body, including:
- Measuring your blood pressure and pulse
- Determining if your vision falls within the 20/40 minimum requirement in each eye
- A hearing test to assess if your auditory acuity is capable of perceiving a forced whisper

If you pass the exam successfully, you will receive a medical card or certificate from the medical examiner. The card usually lasts for 24 months, barring any reasons the examiner requests you to be rechecked sooner.
